Well, the Faculty person gave me the email of a BoGS person (while agree that CamSIS was a nightmare to use). I emailed the BoGS who replied agreeing that picking a term was bizarre and illogical and saying she wasn't sure why it was there. A few moments later, I got the form and things became clearer. The form comes pre-printed with certain information including date applied for. This makes sense on the form; it is helpful for a printed form to have the date (or some indication thereof) it was filled in on. It also had my title (the one I prefer) Middle name (i.e. the one I used) and surname on. This made me more convinced about how CamSIS handles names. I think my conclusion from this is that back end is actually ept but the user interface is horrible!
